COVID-19 VACCINATION FOR PRIVATE OCCUPATIONAL THERAPISTS (PRIVATE CENTRES / FREE-LANCER)

Good day!

OT Malaysia is planning to apply the COVID -19 vaccination from Ministry of Health for all Occupational Therapists at private sectors especially those who are running own centres or freelancers.  This is applicable for all active members only. Please fill up the google form below if you would like your name to be added in the list. Kindly ignore this notification if you already registered with us previously. 

This registration will be opened until 31^st May 2021 only and any request after that will not be entertained. Kindly contact us at secretary@ot-malaysia.my if you should require further assistance. Thank you.
